At Livity Care Ltd we understand that Service Users’ journeys progress at different paces and Service Users need different levels of support along the way. We provide a range of care packages to help Service Users bridge the move from hospitals or residential care home recovery setting to living independently in their own homes.

We provide Supported Housing and trained staff to help individuals to maintain a stable mental health, tenancy, develop independent living skills and access community services. Although we have a standard, core provision, the precise facilities and support we offer is flexible and tailored to meet local needs. We strive to provide a positive, supportive and person centred environment for our customers, their circle of support and all our committed staff teams.

How we create our supported living plans?

Our support plans are only created once we’ve really got to know each person and built a clear understanding of their history, their current situation and their future goals. We have in-depth conversations with the person, their family and any relevant care professionals to make sure that we have all the necessary information and have taken the time to hear each person’s views and wishes before we provide our care. Once a supported living support plan is in place,
we regularly review progress, recognising and celebrating every achievement and continuing to encourage each person to set new goals to achieve even more.

Domiciliary & Live in care

We specialise in working with customers and their families who require care in their own homes. We offer a unique service of personalised care services from check-in visits to full live-in support. We keep you in control and provide you with the home care services and support you want, where and when you want it. It’s your life and your care, so it must be your way. When daily visits aren’t enough, our live-in care service allows you to be well looked after in familiar surroundings. One of our trusted, vetted carers lives with you, or your family member, to provide daily one-to-one care. Having your carer on hand gives reassurance and peace of mind to you and your household. We want you to be able to keep up your favourite daily routines, live with your pet or stay with your family for as long as possible.

Semi Independent Living Service

Living on your own for the first time can be extremely daunting. We know this can be especially
true for our young people who have grown up in the care that’s why we created our Semi Independent Living Service. The service provides accommodation, care and tailored support to help young people leaving care aged 16 to 18 years prepare for independent living and learn valuable life skills at their own pace

About the service

Our homes, provides young care leavers will have their own rooms, communal areas and a shared
kitchen. rust staff provide round the clock (24/7) on-site support, as well as running a life skills programme which includes guidance around general housekeeping, healthy eating, gardening, money management and social interaction.
 
Young people also undertake other programmes that have been co-designed by our care leavers including ‘Keys to my future’ and ‘Keys to my employment’. It is expected that care leavers will stay until they are ready to move on to live in their own home.

How we provide flexibility

We don’t just use paid staff to deliver our support. We also use volunteers, the volunteers know the area well and the people who live there. They can help link people into the opportunities in the area as well as warn them about any concerns.
 
We know that the community has a lot to offer the people that we support and that they have a lot to offer the community. By helping them link in we provide more levels of support, build relationships outside of support and help people take the step to independence.
